Together OT is a private pay practice and is not in-network with any insurance plans and does not bill insurance directly. ​

Together OT can provide a Statement of Services referred to as a 'superbill' that you can submit to your insurance for reimbursement. Level of reimbursement is based on your individual insurance plan.

Questions to ask your health insurance representative about your out-of-network benefits prior to the start of your OT services:

1. Does my coverage include occupational therapy services?

2. Does my coverage include seeing an out-of-network provider?

3. Do I need a prior authorization?

4. Do I need a referral from my primary care physician to access my out-of-network benefits?

5. What forms will I need to fill out?

6. How soon after the evaluation or treatment session will I need to submit the forms for reimbursement?
